I am being provided a WiscMed Wispr digital otoscope for evaluation. In addition to the device, I will be provided with a Welch Allyn battery handle, a carrying case and disposable speculum. I agree to use reasonable care in the handling of the device to avoid damage. I will not be responsible for accidental damage.

I agree to return the device, battery handle and case (but not the disposable speculum) within one week of the end of the evaluation period. I am being provided with a return label and instructions on where to return the equipment.
